VACCINATION REQUIRMENTS All animals that are seen, or kept under the supervision of Double Oak Mountain Animal Hospital are required to be up to date on vaccinations. Dogs are required to have up to date Rabies, Bordetella, and Distemper vaccinations. Cats are required to have up to date Rabies and FVRCP vaccinations. In the event that I cannot provide documentation that my pet is current on these vaccines I, the undersigned, hereby authorize this hospital to administer them and agree to assume the cost of said vaccinations.

FLEAS & TICKS All animals kept under supervision of Double Oak Mountain Animal Hospital will be inspected for fleas and ticks. If any are found on your pet, they will be treated at owner’s expense.

ABANDONED PETS If you have not picked up your pet or communicated with us for 10 days past the above confirmed departure date, your pet will be considered abandoned and action will be taken to either re-home the pet or release them to animal services.

PAYMENT Payment for any and all boarding/treatments is required in full at the time of pick up. Payment prior to the date of pick-up is also welcomed.

MEDICAL AUTHORIZATION I hereby authorize Double Oak Mountain Animal Hospital to provide any necessary medical care for my pet(s) in the event of a medical concern or emergency occurs in which neither I, nor my designated emergency contact can be reached within a timely manner from the start of presenting concerns. I relinquish Double Oak Mountain Animal Hospital of all liability in the even of any medical emergencies that occur as a result of failing to disclose any of my pet’s known medical or behavioral concerns. I acknowledge that I may be held liable if any staff member(s) sustain injury while caring for or handling my pet as a result of failing to disclose any behavioral conditions, such as fear aggression/anxiety/animal aggression/male aggression/etc.
