I understand that this document is an agreement with MVAH regarding My Vet At Home Wellness Care Plan 365 as a preventive care plan that helps cover the cost of your pet’s basic needs through a budget-friendly program by arranging monthly payments.

I authorize MVAH to debit the agreed amount monthly.

I have read to my satisfaction all the services covered by MVAH WCP 365. I understand that if my pet requires medical services not covered by the WCP, there will be an additional cost. I know that if I cancel the plan, I will be responsible for the regular price of the services provided by MVAH up to date.

Terms and Conditions.

There is a one-time initial payment fee of $99.
The plan is non-transferable.
The plan cannot be applied to past visits.
An appointment must be scheduled for services.
House call visits 50% off.
Physical exam 50% off
Veterinary Nurse Home Visit 50% off
Additional costs may apply to services that are not listed.
Additional costs will apply for county license and tag.
Services not used during the year will not be transferred to next year.
A flat fee of $14.12 for disposal of biohazard material will be applied to each visit if any medical procedure is preformed.
Not all medical services and procedures are included, and additional costs will apply.
Depending on the selected plan, there is an additional discount of up to 15% for all medical services provided by MVAH during the year. If third-party services are received, the total cost will be charged. Prescribed medications are not included.
The total amount agreed will be debited monthly.
If by pet parent choice the plan is canceled, all you are responsible for is the regular price of services you’ve utilized thus far.
