Silver Bluff Animal Hospital, LLC (referred to in this agreement as “SBAH”)

I am placing my pet in the care of SBAH, with the understanding that the hospital will use reasonable care to keep my pet in good health.

I understand SBAH will provide dry food for your pet at no charge, if any other food is required or requested you will be charged for that food.

I understand If my pet requires medication to be given while staying at SBAH there will be a daily $2.00 medication fee.

I understand that if my pet requires special or extra care while staying at SBAH there will be a daily $5.00 special care fee.

In case of illness I give my consent for SBAH to stabilize the condition by treatment, prescribing for or operating upon my pet(s) as deemed necessary by the attending veterinarian until such time the I can be contacted. I understand any expenses incurred shall be paid promptly by me.

I understand that a physical exam by one of our veterinarians must be current within one year for boarding and all vaccinations for dogs (Distemper, Adenovirus, Parvo, Rabies, H3N2/H3N8(Flu) & Bordetella) and for cats (Leukemia, Distemper Combination, and Rabies) must be current within one year for boarding. If these vaccinations or physical exam are not current, SBAH will administer/perform them and any expenses incurred shall be paid promptly by me. If a pet received these vaccines at another facility, it is the responsibility of the owner to provide proof of the vaccines. Physical exams performed at another facility does not exclude the pet from needing yearly physical exam by one of our veterinarians to board.

I understand that SBAH strives to be a parasite free boarding facility. Therefore, if my pet has fleas upon arrival, SBAH will administer a flea preventative of their choosing to prevent any possible infestation of fleas during my pet’s stay. If evidence of intestinal parasite(s) is observed, SBAH will treat the animal with a medication prescribed by the attending veterinarian. I understand and agree to pay for the above mentioned product or services.

I understand SBAH is not responsible for lost or damaged items left with a pet. It is not recommended to leave personal bedding items with your pet as all bedding materials are washed several times daily and will not be left in the cage for the entirety of the stay. SBAH will provide appropriate bedding for each animal.

I understand that in the event that my pet soils itself during its stay at SBAH a bath WILL be given and appropriate charges applied. I understand this policy and agree to pay for such service.

I understand that should the circumstances arise that my pet(s) remain unclaimed after the date, which I have stated as the pick-up date, an attempt to notify me will be made. Ten days after such notice the pet(s) will be considered abandoned and property of SBAH. It is further understood that such action will NOT relieve me from paying all costs of services and the use of SBAH.

I understand SBAH is not responsible for animals that climb fences or escape. I will not hold SBAH liable and I assume all risks for the boarding of my pet.

Waiver, release and indeminfication. I release, waive, discharge, indemnify and agree to hold sbah, its owners, directors, officers, employees and agents harmless for any and all manner of damages, injury, claims, loss, liabilties, costs or expenses, attorney’s fees, causes of action or suit, whatsoever in law or equity, arising out of or related to the services provided by sbah, its owners, directors, officers, employees or agents including without limitation: (1) Any inaccuracy in any statement made by myself or information provided by me to sbah, (2) My pet, including but not limited to distruction of property, dog bites, injury, and transmission of disease, and (3) Any action by myself that is in breach of the terms of this agreement.

I have read and fully understand the terms of this agreement. I have signed this agreement freely and voluntarily without any inducement, assurance or guarantee and intend it to be a complete and unconditional release of all liability to silver bluff animal hospital, its owners, officers, employees and agents to the greatest extent premitted by law. I further agree that if any portion of this agreement is held to be invalid or unenforceable, the remainder of this agreement shall remain in full force and effect.
