Client Financial and Responsibility Agreement Forms

Welcome! The Doctors and Staff at Sandy Paws Veterinary Hospital want to Welcome you. We also want to make a commitment to you to provide the highest level of medicine and care in a warm, compassionate, and professional manner.

Client Agreement Information:

All Veterinary Services will be provided by Doctors and Staff of Sandy Paws Veterinary Hospital. Sandy Paws Veterinary Hospital cannot professionally or ethically provide guarantees on the outcomes of any services. We operate under the Veterinarian’s Oath and the limitations of state, federal, and national laws.


I / we will provide information about my animals that is truthful and willful to Sandy Paws Veterinary Hospital.


I / we will be responsible for all preventative care such as routine vaccinations, heartworm testing and prevention, proper diet, water, exercise, and shelter that my pets need.


I / we understand the financial obligation and financial agreement between myself (us) and Sandy Paws Veterinary Hospital.


I / we understand no show and cancellation policies at Sandy Paws Veterinary Hospital.


I / we understand standard media release policies at Sandy Paws Veterinary Hospital.


I / we understand the behavioral expectations of them while seeking services at Sandy Paws Veterinary Hospital.

Financial Agreement:

Sandy Paws agrees to provide written estimates to be signed by all clients in order to be transparent for any services or procedures over Three -Hundred -Dollars (300.00). These estimates are no guarantee or assurance that the price will not increase due to unforeseen circumstances related to your pet.


In admitting my pet(s) for diagnostics, treatments, or surgery I authorize the Veterinarians and staff of Sandy Paws Veterinary Hospital to perform such diagnostics, treatments, or surgical procedures deemed necessary.


I / we agree to pay for all services rendered at the time they are provided.


Sandy Paws Veterinary Hospital will not store credit cards or debit card numbers or any information associated with these cards.


I / we will guarantee that all funds are available at times of services when paying by check. In case a check is returned due to insufficient funds I assume financial responsibility for the returned check and any associated bank fees (Fifty -Dollar / 50.00) accrued by Sandy Paws Veterinary Hospital. I also forfeit the use of paying by check in the future. I / we understand that any failure to rectify any of these fees can result in collections and termination of the veterinarian client patient relationship.


Due to the increase in demand for veterinary services at Sandy Paws Veterinary Hospital and the increased number of clients failing to show for scheduled appointments despite reminder calls / texts/ and emails. I / we understand that a Fifty-Dollar (50.00) no show fee could be placed onto my account if I / we fail to cancel any appointment within 24 hours of arrival. This is nonrefundable.


I / we understand if I / we cancel or no show for any scheduled appointment more than two (2) times that a deposit of Fifty -Dollar (50.00) may be required for any future visits at Sandy Paws Veterinary Hospital. This deposit is nonrefundable and this deposit will be used towards the next available visit.


I / we understand that if I/ we are more than ten minutes (10) late for any appointment at Sandy Paws Veterinary Hospital that It may be necessary to be rescheduled for a later date, and I / we understand that this is in the best interest and care for my pets and others.


I / we understand that a One-Hundred-Dollar (100.00) cancellation fee will be applied to my account if I / we cancel any Surgical, Ultrasound, or Pet Friendly Dental less than 48 hours in advance. This deposit is nonrefundable.

Media Release:

I / we give permission to Sandy Paws Veterinary Hospital permission to photograph / record my animals.


I/ we agree that Sandy Paws Veterinary Hospital may post images or recordings of my pet on their Social Media accounts (Facebook or Instagram).


I / we agree that Sandy paws Veterinary Hospital may use these images for marketing or educational purposes.

I / we are comfortable with my pet being photographed for the purposes above?

Providing you with a high level of care and services requires mutual respect, cooperation, and kindness. To nurture an inclusive and mentally healthy environment for both clients and staff, we have instituted a Code of Conduct for all Sandy Paws Veterinary hospital clients and staff.


Empathy, Understanding, and Patience.


At times we may fall behind and our team works diligently to stay on time but there may be instances in cases of emergencies, unexpected medical crises, and situations out of our control. These instances may need to be prioritized for the individual patient. The individual attention and approach to patient care relies heavily on equitable time management for all.


We have a ZERO tolerance policy for the following:


Verbal abuse, profanity, malicious, harmful statements to any staff members by phone, email, in person or social media accounts.


Any form of harassment such as cyberbullying, discriminatory comments/ actions and intimidation or threats.


Refusal to pay for products or services. This may lead to corrective measures such as being asked to leave the property, termination of the client patient relationship, collections, or police involvement.
