ATTENTION: Please plan to arrive for pick up and drop off by 5:45PM on weekdays and by 12:15 PM on saturdays. All pets picked up or dropped off after 6:00 PM on weekdays and after 1 PM on saturdays will incur a late fee of $35.

List medications that your pet will need while boarding. Supplement and medication administration while boarding comes at an additional cost of $4.00 per night.

FEEDING INSTRUCTIONS

Please list the brand of food you have brought with your pet and feeding instructions. We know boarding can be stressful so we would like to feed your pet the same way you do at home. Unless otherwise specified, your pet will be fed Hill’s Science Diet Sensitive Stomach Adult dry food for no additional cost. There is an additional fee to feed your pet prescription diet not brought with your pet.

If your pet does not eat any food within a 24-hour period, we will provide a can of Hill’s Science Diet I/D wet food for an additional cost; if your pet still does not eat we will reach out to you.

Hospital Services

Does your pet need to see the veterinarian during his or her stay? Please give a detailed description of the medical condition that needs to be addressed including symptoms, location, and duration of the problem.

Boarding Services Include

Twice daily feeding with Hill’s Science Diet Pet Food along with fresh water at all times
Canine guest let outside a minimum of three times a day for bathroom breaks and playtime
Administration of heartworm/flea preventative brought from home
Complimentary bath with a stay of seven or more nights for our canine guests
Play time for dogs including fetch, chase, tug of war, Kong toys, and (water games during the summer)
Play time for cats including toys and laser chase

Please be advised that for the safety of our feline boarders, cats will remain in their kennel unless receiving medical attention.

Vaccinations And Hospital Boarding Policies

All pets boarding with us must be current on vaccinations. If your pet is not current on vaccinations, by signing this form you consent to pay all costs incurred to update your pet’s vaccinations. This policy is for the safety of your pet, as well as the safety of our staff and other guests in the kennel.

Because boarding can be a stressful experience, we occasionally find that a pet becomes ill during their stay. If your pet requires medical attention, by signing this form you give the veterinarians and staff of Southern Crossing Animal Hospital authorization to treat your pet should a medical situation arise. Also, you agree to pay for any charges incurred in treating your pet.

All boarding pets must be free of fleas and other intestinal and external parasites. If any parasites are noted during your pet’s boarding, you agree to pay for the cost of treating your pet for parasites.

Aggressive Animal Fee

In the event a boarding animal becomes aggressive and poses a threat of our staff’s safety, we reserve the right to assess an additional aggressive animal fee. Southern Crossing Animal Hospital will make every attempt to contact you or your emergency contact to give you the option of either paying the additional fee or the opportunity to pick up your pet.

I understand that all fees are due at the time services are rendered. In the event of default or failure to pay, I the undersigned, agree to pay all attorney’s fees and collection cost of said debt.
