I hereby consent and authorize anesthesia surgery for my pet. I understand that my pet will be undergoing anesthesia and that there are risks involved with anesthetic. I understand there is no guarantee or assurance as to the results of the procedure and the condition may not be cured.
I further authorize the performance of additional operative procedures during the course of the surgery, when the veterinarian determines it to be advisable.
An examination must be completed on all pets being anesthetized. Your pet will not be anesthetized until a veterinarian has examined and reviewed the history and current health of your pet.
As part of surgery, your pet will receive injection(s) to help manage pain. These injections will only last 6–8 hours. Certain surgical procedures will require that your pet be sent home with pain relieving agents. Your doctor will make the decision as to what type and how much pain medications will be sent home.
We highly recommend pre-operative blood work to evaluate your pet’s internal health if it has been longer than 5 months since last normal test. This test will determine if the kidneys and liver are functioning well before we anesthetize your pet.
