BOARDING & GROOMING REQUIREMENTS
The following boarding and grooming requirements are for the health and safety of the pets we are entrusted with.
All pets boarding, day boarding, or receiving grooming services at MAH must have had a physical examination by a licensed veterinarian within the past 12 months. This is to ensure that your pet is healthy and does not have any medical issues that need attention while they are in our care. All pets must be vaccinated by a licensed veterinarian and MAH must have proof of these services. If such proof cannot be provided, the owner will be notified and Dr. Ledet will perform an exam and will administer all required vaccines at the owner’s expense. Vaccines that have been administered by a non-veterinarian will not be accepted.
• For puppies and dogs, required vaccinations include: Rabies 1 or 3 year (if greater than 12 weeks old), Distemper/Parvo, Bordetella (recommended biannually), and Leptospirosis. Canine Influenza is recommended, but not required.
• For kittens and cats, required vaccinations include: Rabies 1 or 3 year (if greater than 12 weeks old), FVRCP, and Feline Leukemia.
MAH is a flea free and parasite free environment. All pets must have a negative fecal examination within the last 12 months to ensure our environment stays free of harmful parasites for the health of our guests. All pets must be on a monthly internal and external parasite preventative. All pets will be examined for fleas and if present will receive a Capstar tablet flea treatment upon arrival at the owners expense. If ticks are present, an appropriate tick treatment will be administered.
Even though vaccinations and parasite prevention requirements are in place at MAH, pets are in common areas at times in which communicable diseases may pass between seemingly healthy pets. If following a provided service date a pet shows signs of illness, the client may not hold MAH liable for any inadvertent exposure the pet may have received, and if that client requests an examination, treatment, or medications for the pet, it will be carried out at the owner’s expense.
MAH is to be informed of any concerns or health conditions or stress related issues your pet may have.
In the event of an emergency, Dr. Ledet and MAH will provide immediate medical attention for a pet at the owners expense. All attempts will be made to contact the owner in the event of an emergency.
GENERAL BOARDING INFORMATION
MAH allows for labeled personal items such as certain blankets, toys, and treats to be left with your pet during their stay ONLY if they do not have the tendency to chew things. Please do not bring rawhides, pigs ears, bully sticks, or bones as we do not recommend them for safety reasons. MAH is not responsible for items lost or damaged. MAH provides bedding for all pets in the facility.
There is an additional charge to administer medication provided by owners, and clear and concise instructions must be provided. Ample quantities of medications or special diets should be provided or can be supplied at the owners expense.
If no food is submitted, pets will be fed either Hill’s, Royal Canin Sensitive Stomach or Purina. If a pet is not eating adequately, MAH will offer a canned digestive formula at the owner’s expense.
You may pick up or drop off for boarding between 7:30 am and 5:30 pm Monday through Friday.
A 50% deposit will be collected for all new client boarding reservations as well as holiday reservations and will be required to hold the reserved dates. Deposits paid will be used toward the final payment at checkout. If holiday reservations are not kept or cancelled within 48 hours of the arrival date, the entire deposit is non-refundable.
Pets staying three nights or more will receive a complimentary bath. Pets being picked up before noon may be bathed the day before their departure. Additional spa menu and services are available for an additional cost.
A hurricane boarding form must be completed annually for all pets boarding at MAH during June 1- November 30.

GENERAL GROOMING INFORMATION
MAH and our groomer make every effort to give accurate quotes for charges for grooming services, however, on occasion the nature of the groom may be different based on services requested or the condition of your pet's coat or their behavior. You will be notified if additional charges are anticipated.
Grooming is a process that involves many steps from start to finish. This process may take several hours to complete as each pet is designated generous time for a meticulous groom. MAH provides water and comfortable bedding for all grooming pets as well as many potty breaks during their visit. If you prefer to have your pet stay for only a limited amount of time or need to pick them up by a certain time, please notify us when you make the appointment, as well as let us know when you drop off. Please note, if you do not specify a requested time, your pet may spend the full day here. Drop off time is typically by 9 am.
Enrichment activities and enhancements are available during grooming visits for an additional cost.
MAH requests that new client grooms speak to our groomer directly to discuss estimated cost and specifically what is expected of the groom haircut.
MAH promotes a fear free environment and will continue a groom if the patient is having undue stress despite our comforting efforts. If this situation arises, you will be contacted and if appropriate, a medication may be offered at the owner's expense to help calm your pet and facilitate the completion of the groom in a safe, calm manner.
MAH puts patient comfort above all else. Every effort is made to ensure that bathing and grooming services are a positive and beneficial experience for your pet. Pets with matted coats or overgrown nails into the paw pads require extra attention. In the event that a pet's coat is matted, the groomer may have to shave the matts out rather than perform a painful and stressful de-matting procedure. If a pet is severely matted, there is an increased risk for abrasions or cuts to occur. Matted pets take additional time to groom, so there may be an additional fee added on to regular grooming price if my pet's coat is matted. Heavy matting can also trap moisture and urine near the pet's skin allowing mold, fungus or bacteria to grow, producing skin irritations that existed prior to the grooming process. After matts are removed, certain medical conditions may be revealed or occur, including but not limited to, itchiness, skin redness, infections, self-inflicted irritations or abrasions and failure of the hair to regrow. In some cases, pets may also exhibit brief behavioral changes. Prevention is the best defense by scheduling regular grooming appointments, every 4-6 weeks. MAH is not liable for these conditions and any medical treatments necessary are at the expense of the owner.
Please contact MAH immediately if any medical concerns arise after a bathing or grooming visit. All grooms are subject to a $25 cancelation fee for grooming appointments not kept or cancelled within 24 hours. If two appointments are missed without notice, client's are then required to pre-pay prior to scheduling any future appointments.

I have reviewed this agreement for accuracy and understand the contents of this contract. I affirm that I am the rightful legal owner of the pet(s) for which services are being rendered. I authorize this signed agreement to be valid approval for future boarding and grooming services, as long as I am a client in good standing of Mandeville Animal Hospital. I understand that policies and pricing are subject to change. I have read, signed, and agreed to the above policies and procedures.
